Learning Objectives:

1. Select the appropriate perioperative POCUS exam for a given clinical question and justify that choice based on indications and limitations.
2. Demonstrate probe selection, patient positioning, and machine optimization (depth, gain, focus) to acquire core live-scanning views.
3. Identify normal sonoanatomy and common artifacts during live scanning and differentiate artifact from clinically meaningful findings.
4. Interpret high-yield abnormal patterns relevant to ASRA practice and relate those findings to immediate management implications.
5. Integrate POCUS findings into real-time decision-making and specify essential documentation and quality/safety checks for reliable clinical use.

Learning Objectives:

1. Upon completion of this session, participants will be able to illustrate the unique anatomy and sonoanatomy of the spine, and formulate a perioperative regional anesthesia and acute pain management plan for patients undergoing both open and robotic spinal surgical procedures.
2. Upon completion of this session, participants will be able to describe the unique anatomy and sonoanatomy in children, and formulate a perioperative regional anesthesia and acute pain management plan for pediatric patients undergoing various surgical procedures, particularly in the extremities, chest, and abdomen.
3. Upon completion of this session, participants will be able to understand the complex comorbidities and prioritize the unique needs for regional anesthesia in patients in the intensive care unit, and subsequently formulate a regional anesthesia and pain management plan for these patients who often have competing physiological and treatment needs.

Learning Objectives:

1. PRO: Evaluate the evidence for lidocaine, mepivacaine, chloroprocaine, and low-dose bupivacaine as spinal anesthesia drugs for same-day total joint arthroplasty
2. Discuss the most effective and efficient spinal anesthetic formulations that will minimize adverse events in recovery
3. Identify common causes for prolonged post-operative stay and unplanned admissions in ambulatory surgery
4. Discuss the clinical and operational unpredictability of spinal anesthesia in ambulatory and short-duration procedures, including variability in onset and duration, risk of failure or conversion to general anesthesia, postoperative pain management challenges, and the resulting impact on OR efficiency, staffing, discharge timing, and patient experience

• Discuss the surgical populations that might benefit from perioperative methadone and describe the risks, including patient populations that deserve special attention
• Describe the unique pharmacology of buprenorphine and how acute pain management strategies should be adjusted in patients taking buprenorphine
• Discuss the differences between oral and IV ketamine and how oral ketamine could be advantageous in some clinical settings
• Evaluate the literature on gabapentinoids for acute pain and be able to define the patient populations that are likely and unlikely to benefit from gabapentinoids

Learning Objectives:

1. Summarize the current clinical evidence supporting the use of intrathecal opioids for optimization of postoperative analgesia in adult surgical patients.
2. Compare the analgesic efficacy, risks, and limitations of intrathecal opioids with fascial plane blocks and continuous perineural or neuraxial analgesic techniques.
3. Identify appropriate postoperative monitoring requirements for patients receiving neuraxial opioids based on contemporary guidelines and best practices.
4. Describe how the use of neuraxial adjunct medications in obstetric anesthesia has changed.

1. After participating in this session, participants should be able to discuss the merits and drawbacks of utilizing liposomal bupivacaine in peripheral nerve blockade, and specify the manner in which it may be administered to provide optimal duration of analgesia.
2. After participating in this session, participants should be able to reflect upon the current regional anesthetic practices in the patient with long bone fracture and assess whether the potential risk of compartment syndrome truly precludes the use of this effective analgesic modality.
3. After participating in this session, participants should be able to formulate an acute pain management plan that balances the benefits and limitations of regional anesthesia and systemic opioid and non-opioid analgesia.
4. After participating in this session, participants should be able to debate the potential benefits of motor weakness in regional anesthesia

• Identify and adapt regional anesthesia strategies in the setting of anatomic variability when standard block techniques fail to provide adequate anesthesia.
• Apply regional anesthesia techniques as a diagnostic tool to differentiate sources of acute pain (somatic vs visceral) and guide management during acute pain consultations.
• Explain the physiologic effects of regional anesthesia–induced sympathectomy on tissue perfusion, with specific relevance to graft survival and postoperative outcomes.
• Evaluate the impact of regional anesthesia on surgical and patient-centered outcomes in high-risk or nontraditional clinical scenarios.
• Analyze clinical situations in which regional anesthesia may serve as a life-saving intervention and integrate these principles into perioperative decision-making.
